Care Partner / Case Manager

Visa sponsorship

Community Services · Sydney, NSW, Australia · Full-Time · $80,000 - $95,000/yr · Visa: Employer sponsored

Strong case managers in aged care are hard to find, and this Sydney opening shows it. The employer is an aged care provider that supports older Australians at home through the Support at Home program, and it is recruiting a Care Partner / Case Manager who brings compassion and solid experience.

You will own the client relationship. That means leading care coordination, running individual support plans and making your own calls in a flat team where nobody stands over your shoulder. Every client should get fitting services at the moment they are needed. Visa sponsorship is also available for the right candidates.

Go in prepared. Your English CV should prove the client assessments you have completed, the care plans you have reviewed and the support staff you have supervised. Sharpen your case management CV with Build AI CV before you send it.
